Loyal Citizen's Form


Ripped entirely from the Rude One, I made a little pdf form Loyal Citizens can fill out and mail into the President if they so choose.

Update:
It seems a few people are having trouble with the link above, so I'm trying the GeoCities route. Please forgive my general lack of web knowledge, I hope this works for everyone.

Some "constructive criticism":
the form says you must wait until you're in the presence of an NSA officer before signing. This would of course prevent most people from actually signing it and mailing it in.
You might want to model it a bit more after official government documents, i.e. give it a document number, revision, expiration date, etc. Some good ones to model after are http://uscis.gov/graphics/formsfee/forms/files/i-130.pdf and http://uscis.gov/graphics/formsfee/forms/files/i-765.pdf

Some spelling corrections just at first glance.

"disposed"

"discretion"

"certifying"

Also, just some extra thoughts: I think it would be funny if, in the section where the NSA officer acts as notary public, it included things like "retinal scan," "stool sample," etc.

Anonymous, spell check has been updated. Thank you too.

I thought about the other things in the "constructive criticism" but didn't want it to be so detailed that people could use it for phishing. Soc. Sec. etc.

I actually did use a citizen identification form from a .gov site as a template.

Other than that if people have other ideas, please feel free to borrow or modify my document in any way. I just do this for fun and hope you all do the same.

i've followed the "oath" since it was first posted on the Rude site, and good idea on the pdf thing.

hope you can fix some of the holes (like the misspellings)
